Microscopic appearance:

Rouleaux formation, bicytopenia. Blasts: 79%, Myelocytes: 03%. Large platelets seen. Myelopoiesis and erythropoiesis markedly suppressed. There is diffuse infiltration with heterogeneous population of small to medium sized mononuclear cells having high to intermediate N/C ratio, irregular shaped, agranular cytoplasm. Nucleus having fine chromatin with 2-3 nucleoli. Megacaryocytes suppressed. Lymphocytes and plasma cells decreased. No evidence of granuloma, lymphoid aggregate or fibrosis seen.
